The Peer Outreach Specialist will identify and build rapport with youth and young adults (YYA) experiencing homelessness between the ages of 14-24. Peer Outreach Specialists are tasked with assisting youth and young adults (YYA) in attaining positive and stable housing situations. In collaboration with the Project Coordinator, Peer Outreach Specialists will provide individualized support by helping each YYA develop an Individualized Plan of Action (PoA) based on their needs and strengths to address identified barriers.
DUTIES:
· Conduct Coordinated Entry Screenings
· Perform outreach activities including engaging and developing rapport with YYA experiencing homelessness
· Conduct strengths and needs assessments
· Work with YYA to develop Plans of Action & monitor and evaluate progression (revise)
· Provision of transportation for YYA to necessary appointments
· Provide information, referrals, linkages and advocacy to YYA in accessing services and resources
· Advocate on behalf of YYA and support them to advocate on their behalf
· Ensure thorough and complete records of participants' activity and progress
· Attend community resource meetings, trainings and workshops as required.
· Represent and create visibility for the Pathways to Success program in communities
· Work as active member of departmental team
· Assist with PSND with designated project activities and data collection efforts to assure progress toward goals and objectives
· Other related duties as required or assigned to support project(s) implementation
*This position does require travel, (35% of work will be travel) weekend and/or evening events will occur to meet client and programmatic needs (15%of work will be weekend and/or evenings)*
DESIRED SKILLS/KNOWLEDGE/ABILITIES:
· High School Diploma or equivalent required
· Knowledge of the experience of youth and young adults' homelessness
· Possess a valid driver’s license and vehicle with full liability coverage insurance (transport)
· Maintain regular contact with supervisors and attend all mandatory meetings
· Communicate effectively over the phone and in person
· Required travel for in state meetings
· Ability to sit or stand for long periods of time depending on event
· Ability to exercise good hand-eye coordination, arm, hand, and finger dexterity and visual acuity to use monitor, keyboard, as well as to operate other office equipment and read information
· Perform other duties as assigned
